Millions drop health coverage as premiums spike

After enhanced ACA subsidies expired at the end of 2025, what many enrollees paid for premiums doubled or more, and marketplace enrollment fell from about 23 million to 19.2 million by February 2026.

What happened

It was the largest annual enrollment decline since the exchanges launched in 2014. In a KFF survey, 9% of 2025 marketplace enrollees had already become uninsured.

Where it stands

A shared failure with Congress: the House passed an extension with 17 Republican votes, but the Senate rejected it.

The administration's side

Trump sent mixed signals on extending the subsidies. The administration is proposing plans it says will lower premiums, though critics warn of family deductibles as high as $31,000.
